I, ________________________________ agree to have eyelash extensions applied to my natural eyelashes and/or removed and retouched. By signing this agreement, I consent to the placement and/or removal of the eyelash extensions by the certified eyelash extension artist.
____ I, understand that this is MY consultation and during MY appointments, that I will refrain from the following: bringing guests (absolutely NO Children), smelling like marijuana, being under the influence of narcotics/alcohol, bringing firearms onto the premises, negative/erratic behaviors and cell phone conversations. At anytime I violate the aforementioned, I understand that I will be asked to leave the premises, my appointment will be canceled, my deposit will be forfeited, and I will have to find another lash artist.
____ I understand that in rare occasions there are risks associated with having artificial eyelashes and eyelash extensions applied to or removed fro, my natural eyelashes. I further understand that in rare cases as part of the procedure, eye irritation & discomfort could occur. Irritation could possibly occur as it relates to the adhesive, gel pads, or tape. I agree that if I experience any of these conditions with my lashes, that I will contact the certified eyelash extension professional that performed this procedure & it may be beneficial to have the extensions removed.
____ I have revealed and disclosed to my eyelash extension professional all conditions and circumstances regarding my health and health history, medications being taken and nay past reactions to products used or medications taken. Additional conditions could occur or be discovered during or after the procedure, which could affect my ability to tolerate the procedure.
Should the section that includes 24 hour curing process… be split into another section?
____ I understand & agree to the after-care instructions provided by my eyelash extension professional artist as it relates to the use & care of my eyelash extensions. I realize & accept the consequences of failure to adhere to these instructions may cause the eyelash extensions to fall out and/or decrease the longevity of the lashes. If I participate in any of the following activities prior to the 24 hour curing process, I understand & accept both risks and costs associated with my negligence: water, sweat, steam, swimming, sauna use, pulling on lashes, using oil based or waterproof cosmetics, mechanical curlers, or crimpers.
____ I understand and consent to having my eyes closed and covered for the duration of anywhere between 65-125 minutes, during the procedure. Times may vary depending on the type and number of lashes applied.
____ I hereby consent to the eyelash extension procedure at my own risk. If at anytime I am uncomfortable with the eyelash extension procedure, I will inform my artist and she will use good efforts to rectify the issue/concern which may include ending the session if either party wishes to do so. If my artist is uncomfortable applying lashes to me, she will discuss her concerns with me and may end the session if necessary. I acknowledge that I have received no guarantees, warranties, promises, or commitments regarding the application process or the products used or applied therein or other statements as to the results of this service.
____ I, the undersigned client, acknowledge that there are no refunds, but the lash artist will do her best to address the concerns of you the client.
____ I give Another Level 3D Luxury Eyelash Extensions the absolute right and unrestricted permission to take, use, and display photogenic images of me, through any form of media, print, digital, electronic, broadcast, or otherwise , at any location for art, advertising, media release news articles, marketing, publicity, or any other lawful purpose. I waive any right to royalties or other compensation. I release and agree to hold harmless Another Level 3D Luxury Eyelash Extensions and my artist from any liability in connection to taking or using said images.
____ I, THE UNDERSIGNED, HEREBY FULLY RELEASE, WAIVE, COVENANT NOT TO SUE, AGREE TO HOLD HARMLESS, AND FOREVER DISCHARGE my artist from any and all liabilities, demands, claims, losses, injuries, or damages including court costs and attorneys fees and expenses, of any kind arising out of, or relating to, the application of semi permanent eyelash extension products. EVEN IF, THOUGH CAUSED IN WHOLE OR IN PART BY A PRE EXISTING DEFECT, THE NEGLIGENCE, WHETHER SOLE, JOINT, OR CONCURRENT, GROSS NEGLIGENCE, STRICT LIABILITY OR OTHER LEGAL FAULT OF THE ARTIST.
____ I, the client, certify that I have read, had explained to me and fully understand the above Waiver and Release and Consent form. I have consulted with my artist and have read all applicable literature given to me. I understand the benefits and the potential complications/risks and I am willing to participate in this procedure.
